ラクーンシティで起きた『アンブレラ事件』。最大手製薬会社アンブレラ社が生み出したｔ−ウィルスには生物を凶暴化させる効果があった。しかし、このウィルスが感染拡大したことにより住民らがゾンビ化したため，米国政府はミサイル攻撃による＜滅菌作戦＞を実行し，ラクーンシティは消滅することになった。忌まわしい事件から７年。惨劇は再びアメリカ中西部工業都市の空港で起ころうとしていた。バイオテロや薬害被害者救済を行うべく，ＮＧＯに所属していたクレア・レッドフィールドは空港利用客の中にゾンビを発見。警備員が噛まれたことを発端に次々と一般客や空港職員が感染していき空港内は大パニックとなる。時同じくして，空でも旅客機内でウィルス感染者が出たことで緊急着陸するべく空港へ向ったが，パニックなっているロビーへ激突してしまう。さらに機内から降りてきた乗組員や乗客らは皆ゾンビ化していた。一方，合衆国大統領は直轄のエージェント，レオン・Ｓ・ケネディに任務を授け，事態鎮圧の特別指揮官として空港へ向かわせるが…
