알렉스와 그 일당은 마약, 폭력, 강간 등 나쁜 짓을 일삼고 다니며 그 행동에 대해 전혀 죄책감을 느끼지 않는다. 무리의 우두머리인 알렉스는 유난히 음악, 특히 베토벤의 교향곡을 좋아한다. 그의 친구들은 알렉스가 항상 주도권을 잡는 것에 대해 불만을 느낀다. 외딴 농장에 돈많은 여자가 혼자 산다는 얘기에 알렉스는 동료들과 농장을 털러 가고, 주인 여자와 싸우다 그녀를 죽이게 된다. 도망치려던 알렉스는 친구들의 배신으로 경찰에 체포되고, 살인죄로 14년형을 선고받고 감옥에 수감된다.
감옥에서 알렉스는 ‘루도비코’란 새로운 죄수 갱생 프로그램에 대해 알게 되는데, 간단한 갱생 치료만 마치면 사회로 복귀할 수 있다는 말을 듣고 실험의 대상자가 되기를 원한다. 마침 루도비코 프로그램을 강력히 지지하는 내무부장관이 교도소를 방문하고, 알렉스는 실험 대상으로 발탁되어 루도비코 센터로 이송된다.
