At the end of production for Fullmetal Alchemist: Brotherhood, producers decided to agaim create a feature-length film based on the story line of the anime series. However, unlike Conqueror of Shamballa, the first Fullmetal Alchemist animated film, The Sacred Star of Milos is an action-packed spin-off story that takes place during the events of Brotherhood and away from its usual settings. Written by novelist Yuichi Shinbo (who has written several Doraemon feature films as well), The Sacred Star of Milos follows the Elric Brothers' hunt for a fugitive. During the chase, they rescue a young alchemist, which leads to them taking part in a rebellion happening in the young alchemists' hometown.

First Press Edition comes in packaging featuring illustrations by manga artist Arakawa Hiromu and animator Konishi Kenichi. It includes audio commentary, making of, a key visual pinup, and a booklet.
