24歳の医学部生クラークは、インターンとしてある精神病院施設へやってくる。事前に施設の調査をして改善点を指摘するクラークだが、責任者のフランクス博士は精神病患者を扱う難しさを知らぬ若僧クラークに、実践の場は厳しいと警告する。クラークの祖父が精神病理学界に大きな影響を与えた大人物ということへの嫉妬か、威圧的なフランクス博士にクラークも反感を抱く。施設スタッフには、30年以上も勤務している医者のモートン、ダグラス、看護婦長ヘンドリックス、インターンから正式採用された25歳のサラがいる。クラークは、サラの案内で重度患者を収容している地下へ行く。整然と並ぶ独房、奇声、悪臭、ものものしい隔離状態—「狂人の館」と呼ばれる地下の、上階の通常病棟とは異なる異様な雰囲気に、楽観的だったクラークも不安を覚える。暴れる患者を抑えるドレイクは、患者を人とも思わぬ扱いをし、暴力で制する。クラークの耳に何かを削る耳障りな音が飛び込む。サラには聞こえないようだが、その音は一番奥の44番独房から聞こえてくる…。そして、次々と謎の死を遂げるスタッフたち…。スタッフ殺しの犯人は〜そして44番独房には誰かが存在するのか…。
