明萬歷二十九年，朝臣與萬歷帝的立儲之爭愈演愈烈。鄭貴妃為立兒子朱常洵為太子而謀害皇后，卻陰差陽錯促使朱常洛登上太子之位。京城散步一封謠傳廢掉太子，改立三子朱常洵為太子的匿名書。鄭貴妃把目標引向太子，動搖太子地位。萬歷帝卻為平息輿論，找來落魄秀才皦生光頂罪，強行了結“妖書案”……
萬歷四十八年駕崩，同年太子朱常洛登基。鄭太妃投其所好，又進獻了八名美女。朱常洛好色成性，沉湎於女色，終於體力不支而病倒。鄭太妃用虎狼之藥謀害朱常洛，同時心懷不軌的魏忠賢也向朱常洛進獻“仙藥”。朱常洛病入膏肓，不日，駕崩。這就是歷史上的“紅丸案”……
本劇以明朝晚期的“妖書案”、“詛咒案”、“梃擊案”、“紅丸案”、“移宮案”等幾大疑案為線索，以立儲、皇權交替為關鍵點，講述了明朝萬歷、泰昌、天啟年間二十年中圍繞宮廷的權力鬥爭，表現出君臣、父子、夫妻、同僚在權力面前交錯的黨爭關系及體現出來的復雜情感，描繪出明朝危在旦夕的夕陽晚景。
