From Tetsuo: The Iron Man to Tokyo Fist, Japanese cult director Tsukamoto Shinya has built his career on subverting and shocking through grotesque subject matters, graphic images, and over-the-top violence. His films take viewers on lush, surreal journeys to the root of human nature, however ugly and disturbing that may be. Tsukamoto's latest film Nightmare Detective (a.k.a. Akumu Tantei) is a paranormal thriller exploring the murky subconscious where dreams and reality collide. The film is as dark, violent, and remarkably imaginative as can be expected from Tsukamoto, who himself appears in the film as the root of evil. With a sequel already in the works, Nightmare Detective is also Tsukamoto's most accessible and commercially friendly entry to date, with singer hitomi and popular arthouse actors Matsuda Ryuhei and Ando Masanobu leading the cast.

Detective Kirishima Keiko (hitomi) is investigating two mysterious deaths. In both cases, the subjects apparently commit suicide in gruesome fashion shortly after dialing "0" on their phones; even stranger, a witness claims that one of them seems to have been attacked in his dreams. Although veteran cop Sekiya (Osugi Ren) dismisses the cases as suicide, Keiko, along with detective Wakamiya (Ando Masanobu), decide to investigate further. Believing that the key to the mystery lies in the dreams, they try to enlist the assistance of Kagenuma Kyoichi (Matsuda Ryuhei), a troubled young man whose psychic powers, amongst them the ability to enter dreams, have earned him the name "Nightmare Detective". When he refuses to help, Keiko, desperate for an answer, decides to dial "0" herself.

平靜的日本東京，突然連續發生多起離奇殺人事件。每一件案子都發生在密室之中，每一個死者都死於刀傷，怪異的是他們全都是自己砍傷自己。死者在死前也都會用手機，發送簡訊給同一個特定的號碼。目擊者指稱受害者人「彷彿在夢中，被某人所襲擊一般」。

女警霧島慶子(hitomi飾)臨危受命，負責偵辦這些詭異的殺人事件。她推測，幕後的兇手透過電話，對死者送出訊息，造成催眠的效果。追查過程當中，霧島慶子發現，竟然有人擁有能夠進入別人夢中的特殊能力，這種人俗稱為「惡夢探偵」。雖然半信半疑，霧島慶子還是找到了惡夢探偵(松田龍平飾)，並且希望藉由惡夢探偵的幫助順利破案。

只不過，進入別人夢中是非常危險的。進入了別人的夢中，就會窺視到這個人的本性，被窺視的人心中的傷痕也會殘留在惡夢探偵心中。背負了太多秘密與傷痛的惡夢探偵，漸漸開始厭倦進入別人的夢，頑固地拒絕對協助調查。霧島慶子究竟能不能找出真兇？惡夢探偵到底能不能查出真相？這一切，都像是永遠無法解開的謎…
