The story follows a group of heroes forced by a mixture of circumstance, righteousness and obligation to join the fight to bring down the despotic, corrupt and murderous Emperor Yang of the Sui dynasty, establishing the Tang dynasty in the process.

　　本劇取材自小說《隋唐演義》。濟南府馬快班頭秦琼為人正直，武藝高超，因遭陷害吃上官司，被押解北平府，在表弟羅成等幫助下，公堂之上竟意外與北平王羅藝認親。

　　宮廷之中，伐南陳立下戰功的晉王桶廣結黨營私，設計陷害太子桶勇，弒父殺兄弟，假扮程咬金詐登州投官。誰知卻被愛才楊林認出，收為義子十三太保，楊林還將女兒玉邵主許配給秦瓊。誰知新婚當夜，秦瓊卻得知楊林正是自己的殺父仇人，黯然離去。

　　各路英雄為秦母賀壽齊聚賈柳樓，楊林得知消息帶兵來剿。程咬金、尤俊達不幸被抓。秦琼等人被逼揭竿而起，救出兩位兄弟，舉義旗，直奔瓦崗營寨。程咬金被擁戴為混世魔王、大德天子。楊林帶各路兵馬來滅瓦崗，卻被瓦崗軍幾次打退，瓦崗聲勢大振。

　　楊廣為觀琼花挖運河下楊州，勞民傷財，百姓怨聲載道。十八反王聯合截殺楊廣，但因有李元霸保駕，反王兵敗。程咬金魯莽單騎下楊州，被生擒活捉，李密刑場救下程咬金，程咬金答應禪讓王位給李密。為將反王一網打盡，楊林布一銅旗陣，準備畢其戰於一役。秦瓊英勇破陣，楊林挑撥各反王自相殘殺。與此同時，李世民審時度勢，勸父李訓自立為王。

　　宇文化及殺死楊廣得玉璽逃走，半路卻被程咬金奪下玉璽。程咬金將玉壐交予李密，李密卻不惜用玉璽同竇建德換回蕭美娘，此舉使得瓦崗大將紛紛出走另投他主。徐茂公為李唐招來瓦崗離散的將領，秦瓊、程咬金歸順李唐，又收服尉遲恭。最終，在秦瓊等人的協助下，李世民戰敗其他反王，唐朝實現大統。
