明末秦淮名妓董小宛（尤敏）清麗絕俗，素與冒辟疆（田青）相好。滿清入中 原之後，降將洪承疇（喬宏）伺機弄權，親往南京游說董小宛下嫁攝政王多爾袞（羅維）。洪承疇藉詞多爾袞殘暴不仁，一手策劃揚州十日、嘉定三屠等慘劇，希望董小宛以天下蒼生為念，伺機除去多爾袞，為民除害。董小宛不虞有詐，慨然答允。洪承疇又命甥女帶子（劉小慧）為侍女，監視董小宛。進京之後，洪承疇密報太后（夷光），謂多爾袞私蓄漢女。太后素與多爾袞有私，勃然大怒，急召董小宛入宮，相陪其子順治帝（趙雷）。順治帝深愛董小宛賢慧溫婉，太后正中下懷，遂命董小宛拜滿族大臣鄂碩（魯奇）為義父，從董鄂氏，並冊立 為皇貴妃。董小宛獨擅椒房，皇后妒恨不已。一年後，董小宛誕下一子，封榮親王。
